Year 5 & 6 reviews:
The reviews in Years 5 and 6 are important
because at the end of Year 6 your child will be moving to Secondary School.
The review at the end of Year 5 should discuss the issues and concerns
around the move and the available options for Secondary Transfer.
Year 9 reviews: The
Transitional Review
The review of your child's Statement in Year
9 will include drawing up a Transition Plan. It is a time when everyone
involved with your child (including your child) starts to think about
what will happen when s/he leaves school.
Year 10 & 11 reviews:
The Transition Plan should be reviewed, along
with the young person's Statement. At Year 11, which is the final school
year, the Connections Service should ensure that an assessment is made
of the young person's needs on leaving school and that levels of support
are identified.
